2008 Qualifying Tournament

Only applicants that have played in the Qualifying Tournament in the last 5 years (2003-2007) may submit an on-line entry. If you have not played in the last 5 years, a hard copy application must be submitted prior to the entry deadline with all of the required documentation and information.
